Submitting this form hereby holds harmless all participant golf courses, their employees as well as the Bristol County Junior Golf League, its Board of Directors, Officers, Committee Members, Managers, or any of its league or team sponsors or other parties associated with the Bristol County Junior Golf League from any injury, loss, damage or legal liability, or any injury or damages which may occur to my child while participating in any event sponsored by the Bristol County Junior Golf League.

Submitting this form also gives permission to all participant golf courses, their employees as well as the Bristol County Junior Golf League, its Board of Directors, Officers, Committee Members, Managers, or any of its league or team sponsors or other parties associated with the Bristol County Junior Golf League to use any media of my child for social media, the Bristol County Junior Golf League website and all other parties associated with the Bristol County Junior Golf League.
